Transitional Housing Services
1. Introduction
Welcome to the House of Transitional Opportunity Outreach Program, Inc. (HTOOP) Transitional Housing Program. By enrolling in this program, residents agree to the following terms and conditions, which are designed to ensure a safe, respectful, and structured living environment for all participants.
2. Eligibility
Residents must be 18 years of age or older and meet the eligibility requirements established by HTOOP. Transitional housing services are open to individuals seeking stability and support due to homelessness, reentry, recovery, domestic violence, or other qualifying life transitions. All applicants must complete an intake assessment and submit any required documentation before acceptance.
3. Program Commitment & Residency Expectations
Residents are expected to: Commit to the duration of the agreed-upon housing period. Follow all program rules, including curfews, check-ins, and required meetings. Participate in case management, life skills sessions, or other support services as outlined in their individual service plan. Failure to comply may result in program termination and dismissal from the housing facility.
4. Conduct & Community Standards
To ensure a positive living environment, residents must: Treat staff, peers, and property with respect and care. Refrain from any violent, threatening, abusive, or disruptive behavior. Abstain from the possession or use of drugs, alcohol, or weapons on the premises. Violation of these standards may lead to disciplinary action, including immediate dismissal.
5. Confidentiality
All personal information shared with staff and within the program will be kept confidential, except as required by law. Disclosure may occur in cases involving: Threats of harm to self or others. Reports of abuse or neglect. Legal or court-mandated reporting
6. Fees & Financial Responsibilities
Residents may be required to contribute to program fees or rental contributions based on income or funding guidelines. All fees must be paid on time as agreed. Refunds are not provided for voluntary departure, rule violations, or missed services unless otherwise approved by HTOOP.
7. Completion & Transition Planning
Residents who meet the goals outlined in their service plan and comply with program expectations may receive support with transitioning to permanent housing. Successful completion may include a letter of completion, referrals, or continued case management support.
8. Liability Disclaimer
House of Transitional Opportunity Outreach Program, Inc. and its staff are not responsible for personal actions taken by residents outside of the program or facility. Transitional housing support does not substitute for legal, financial, or medical advice.
9. Program Policy Updates
HTOOP reserves the right to update or revise these terms as needed. Residents will be notified of any significant changes.
